This service provides an outpatient services to people who have fallen or who are at risk of a further fall. The aim of the service is to reduce the risks of falls and fractures to maximise their physical and mental health and well being.
The person must be over 18 years old, registered with a South Warwickshire GP, at high risk of falling and presenting with other significant risk factors following the completion of a risk assessment and medically stable, willing and motivated to conduct exercises for a period of seven weeks and able to mobilise with supervision using a walking aid and attend an outpatient clinic or defined location.
GP, medical professional,carer,user but screened and assessed against the falls Risk assessment (available from the team or website). There may be a waiting list for this service.
